The Privacy Policy isn't a form you fill in. It is, however, a form which explains the privacy policies of Dr. Kurey's office.

New Patient Form
The New Patient Form is provided so that our new patients may print the form and then enter the requested information before coming in to see Dr. Kurey; at the patients' leisure.
